Overview:

We are seeking a detail-oriented and compassionate Discharge Specialist to join our mental health private practice team. The Discharge Specialist will play a crucial role in ensuring smooth transitions for patients leaving our care by tracking and monitoring discharges, facilitating completion of discharge paperwork, and ensuring accurate documentation in patient charts.

Responsibilities:

• Monitor and track patient discharges, ensuring timely and appropriate transitions out of our care.

• Communicate effectively with clinicians and administrative staff to coordinate discharge processes.

• Facilitate the completion of discharge paperwork, including necessary forms and documentation.

• Ensure accuracy and completeness of discharge documentation in patient charts and electronic medical records (EMR).

• Collaborate with insurance providers and other healthcare professionals as needed to facilitate discharge planning and continuity of care.

• Provide support and guidance to patients and their families during the discharge process, addressing any concerns or questions they may have.

• Maintain confidentiality and adhere to HIPAA regulations in all aspects of discharge planning and documentation.

Qualifications:

• Bachelor’s degree in social work, psychology, healthcare administration, or related field (preferred).

• Experience working in a healthcare setting, preferably in mental health or behavioral health.

• Strong organizational skills with the ability to manage multiple tasks simultaneously.

• Excellent communication and interpersonal skills, with a compassionate and patient-centered approach.

• Knowledge of discharge planning processes and familiarity with relevant regulations and guidelines.

• Proficiency in using electronic medical records (EMR) and other healthcare software systems.

• Ability to work effectively both independently and as part of a multidisciplinary team.
