Job Overview:
This role allows for a great deal of autonomy, allows you to own the patient care and serve as a leader within the facility you support. You will collaborate closely with the attending physicians, facility leadership team and other providers with the goal of improving outcomes and the quality of care for residents.
· Coordinate patient care with the facility Director of Nursing and attending physicians, including through proper documentation in the appropriate EMRs
· Function as the primary provider and treat an average of 12 to 16 patients per day
· Take initiative and collaborate with other BridgeCare providers and partners to ensure patients are getting the best care possible
· Provide care for patients in a dedicated facility by building trust and rapport with facility personnel and attending physicians
· Feel confident making high-risk decisions in the face of ambiguity
· Nurture open communication and teamwork with the facility team and your manager to ensure the success of clinical programs
· Comfortable with the ever-changing environment of a senior care facility
Key Requirements:
As a BridgeCare Nurse Practitioner, you will be a leading member of a multidisciplinary clinical team treating patients at your designated facility, applying a wide array of clinical knowledge. You will work closely with the facility Director of Nursing and Attending Physicians to coordinate patient care. This is a fast paced, ever changing environment is challenging, but incredibly rewarding!
· Nurse Practitioner License
· ANCC or AANP Certification
· Ability to work in the U.S.
· Experience in a nursing home space is a plus
Typically, the APN is onsite, daily as a full-time practitioner, working a standard Monday – Friday work week with flexibility in the 40-hour work week. Providers take call on a rotating basis between 5p – 9a the following day for their building group. Providers are not expected to go into a facility when on call. Call shifts typically happen once every 7 – 10 days.
