Options Treatment Programs is looking for a full time Community Partnership Outreach Program (CPOP) and Bridging Case Manager.

The CPOP and Bridging Programs utilize a case management/wraparound (strength-based, trauma-informed, person-centered, interdisciplinary) approach to support individuals linked to the criminal justice system.

This position requires the candidate to work from our agency office in Appleton, WI 5 days per week.

Opportunities for this position include:

• Support individuals who are working to gain access to counseling services

• Screen individuals for case management needs.
• Build a service plan around the identified needs.
• Meet with individuals to work toward service plan goals.
• Services to clients are provided by means of phone calls, one on one meetings, family and/or group sessions (in person/telehealth).
• Assist a licensed professional in facilitating curriculum-based groups such as Cognitive Behavioral Interventions for Substance Abuse (CBI-SA)
• Maintain accurate and timely records of services provided
• Collaborate within multidisciplinary team alongside community partners
• Thrive in a fast-paced environment working with a variety of individuals

Qualified candidates must meet the following requirements:

• Bachelors Degree in Social Worker or Criminal Justice
• Previous experience working with correctional clients, wraparound services, and case management experience preferred.
• Possess excellent organizational, time management, assessment and case conceptualization skills.
• Ability to work as part of a cohesive team in a faced paced environment.
