Case Manager Supervisor - Buckman

  • Positions Summary:

    The Case Manager supervisor position is responsible for the oversight of the Case Management team, and provides support to Program Management in oversight of the program operations. The Case Manager Supervisor will facilitate and coordinate case management and documentation completed by other Case Managers, and assist the Program Manager in onboarding and training the Case Management team. In addition to providing leadership support for the Case Management team, the Case Manager Supervisor will assume regular Case Management duties, including overseeing the development of the treatment plan, its implementation and all subsequent reporting and treatment plans for up to 3 clients. This position must facilitate effective communication and working relationships among fellow Case Managers, youth care specialists, and the management team to ensure timely implementation of services and effective operations of the program.

    Education and Experience Requirements:

    • Bachelor’s degree with major study in social work, psychology, sociology, or a closely allied field and two (2) years of experience in the care and rehabilitation of youth OR a Master’s degree with major study in social work or a closely allied field and one (1) year of experience in the care and rehabilitation of youth.
    • At least 1 year Case Management experience required.
    • Previous supervisory experience preferred.
    • Working knowledge of BRS standards preferred.
    • Prefer experience working with Oregon Youth Authority.

    Required Knowledge, Skills and Abilities:

    • Genuine concern for young people and the ability to treat all people with dignity and respect, and openness to learning and implementing a Positive Youth Development approach to youth work.
    • Ability to work effectively within: culturally diverse populations; team dependent models; crisis management; strength-based theoretical orientation; adolescent development; and family reunification.
    • Ability to provide comprehensive leadership and support to a team.
    • Knowledge of stages of change, motivational interviewing, and trauma informed care.
    • Knowledge of community resources that support and promote independent living skills.
    • Knowledge of Oregon Youth Authority, Department of Community Justice, and/or other systems of care for adjudicated youth.
    • A commitment to on-going training and a willingness to receive and act upon feedback.
    • Ability to accurately read, record, and interpret information and complete written documentation that meets contract and program guidelines.
    • Ability to provide visual and auditory supervision of program and youth.
    • Ability to communicate effectively orally and in writing with a broad variety of people, including youth, families, staff, supervisors, and community partners.
    • Ability to stay calm and emotionally available in an environment serving young people who require a high level of supervision and redirection from staff.
    • Ability to exercise independent discretion and judgment within program guidelines.
    • Ability to work effectively in a team-dependent environment and to act on the agenda of the program.
    • Ability to maintain appropriate professional boundaries working with youth and in handling confidential information.
    • Ability to climb and descend at least two flights of stairs.
    • Ability to escort and supervise clients in the community which may require standing and walking up to an hour at one time.
    • Ability to stay awake and alert throughout entire assigned shift and to perform routine dorm checks and facility walk-throughs.
    • Ability to effectively monitor safety and demonstrate group management skills and effectively de-escalate potentially chaotic or volatile situations.
    • Ability to regularly attend scheduled shift, be punctual for scheduled shift and meetings, and be in a condition suitable for assuming responsibilities of position.
    • Manual dexterity allowing performance of routine office tasks such as writing, phone use, filing, copying, and use of computer keyboard.
    • Manual/physical capability to bend, stoop, kneel, and reach, as well as occasionally lifting up to 20lbs (including overhead) in order to perform functions such as accepting donations or unpacking groceries.
    • Ability to perform and/or supervise routine household tasks such as cleaning, cooking, washing and folding laundry, emptying trash, sweeping/mopping, etc.

    OTHER REQUIREMENTS:

    • Personal transportation allowing for travel within the Portland metropolitan area is required; a personal vehicle for travel and/or transport of clients is preferred. All employees who drive for work purposes must have and maintain a valid driver’s license for state of residency, have a driving record that permits coverage under the corporate auto liability policy, and complete the agency’s driving approval process before driving for work. If driving a personal vehicle for work purposes drivers must also have personal auto liability coverage ($100,000/$300,000 levels strongly recommended). Employees are not to drive for work purposes until they have received written authorization to do so.
    • Bloodborne Pathogens training is required on first day of employment and annually thereafter.
    • Must complete Mandatory Abuse Training within the first 30 days of employment and annually thereafter.
    • Provides emergency first-aid, as needed, and must have and maintain standard first-aid/CPR certification.
    • Must pass criminal history background check.
    • Food Handler's Certification required within two weeks of hire.
    • Must obtain National Provider Identification Number through the National Plan and Provider Enumeration System within 5 days of hire.

    Duties and Responsibilities:

    • Provides oversight and coordination of Case Management team.
    • Coordinates and facilitates onboarding of new Case Managers, and ensures current Case Managers are up to date in training and skills.
    • Monitors and audits documentation of other Case Managers for quality, timeliness, and compliance to OYA and agency standards.
    • Coordinates and leads meetings for Case Management and Clinical team.
    • Communicates with direct care staff, the clinical team, management, and OYA to ensure the implementation of high quality BRS services.
    • Assists Program Manager in running daily operations of program.
    • Assists in coordinating and implementing the behavior management system.
    • Assists in enhancing existing policy and procedure, and creating new policy and procedures.
    • Provides guidance, leadership, and training to entire program team.
    • Coordinates Team activities in relationship to the service planning and implementation of said plan.
    • Insures that intake documentation is complete and client file is set up at intake.
    • Oversees client and parent orientation to facility, staff, and house residents.
    • Ensure written service plans are complete within time allowed in accordance with funding contract standards.
    • Coordinates the 90-day service review
    • Facilitates review meetings with Oregon Youth Authority.
    • Coordinates the discharge planning for each client including completion of termination and aftercare reports.
    • Provides case management for clients.
    • Ensures that the BRS contractual required hours of direct care services are offered and documented in accordance with the clients’ master service plan.
    • Conducts weekly goal planning with each client.
    • Assures involvement of caseworker, court worker, other significant resources, family, client and other staff in post-placement planning.
    • Oversees and coordinates implementation of assigned case management responsibilities for clients.
    • Serves as primary communication liaison between the program and other agencies and the family or aftercare resources.
    • Assumes responsibility for group skills training using established program structure.
    • Works within the team to insure group skills training using established program structure.
    • Provides direct supervision and creates a supportive and nurturing therapeutic milieu.
    • Accompanies clients to medical (including psychiatric) appointments; participates or provides information as needed.
    • Provides emergency first-aid as needed, and maintains a current standard first-aid certification.
    • Provides emergency pager coverage on a rotational basis with other staff within the Programs’ leadership team. This involves responding to crisis calls and proving on-call support, role modeling, and in-person availability if needed.
    • Models and encourages direct, professional communication among staff members and facilitates development of a positives staff support network which honors and encourages the growth and development of all staff, clients and families.
    • Actively supports and seeks to improve programs in compliance with the Janus mission statement.
    • Performs other program related duties as assigned by Supervisor or Program Director.

    WORKING CONDITIONS:

    • May involve exposure to communicable diseases, including a variety of illnesses and infections such as the common cold, TB, Hepatitis, flu, meningitis, and HIV.
    • May involve exposure to upset, angry, severely traumatized or emotionally disturbed children, adolescents, adults, and families.
    • May involve time in excess of routine schedule and/or overtime, including evening/overnight, weekend hours, and/or holidays.
    • May involve exposure to information that graphically describes physical, sexual and emotion abuse of children and may experience vicarious trauma.
    • May require working and providing services at locations other than program site (i.e., schools, café’, clinics, etc.,) and may, therefore, be exposed to environments not under agency control (i.e., second-hand smoke, varying hygiene practices, etc.).
