  • OVERVIEW OF THE ROLE / JOB

    The primary responsibility of your position is to provide care to the Villagers living in a home by protecting, sustaining and nurturing the Villagers , providing assistance with activities of daily living and meeting other needs as required.

    The core goal of the Village Care Provider's (CNA) work is to provide assistance in a manner that promotes a high quality of life and a positive community .

    All responsibilities will follow the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities.

    You are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

    KEY RESPONSIBILITIES :

    • Working with your household team of Village Care Providers (CNA) , recognize and respond to the needs and desires of the v illagers
    • Work various houses or work flexible schedules as agency and supervisor deems necessary. Staff attendance and promptness to assigned shifts is essential.

    The responsibilities of this position may require staff to be able to work irregular, extended, and / or be available for on-call work hours, evenings, nights, weekends, holidays, etc.

    • Promote autonomy of v illagers in decision-making regarding their daily lives and the life of the house
    • Work together with the other Village Care Provider (CNA) as a self-managed team, responsible for managing all aspects of the home, including food ordering, preparation and clean up, light housekeeping duties, all laundry needs of the villager s, and assisting villager s in engaging in the daily activities of their choosing
    • Cooperate with the nurses and other clinical team members to ensure the well-being of each villager
    • Receive and report resident status at the beginning and end of each shift and as needed to team members and to nurses and clinical support team members
    • Assist v illagers with personal care functions, including : dental and mouth care, bathing, dressing and grooming, hair care, nail care, shaving, bowel and bladder care, other personal and hygiene care
    • Assist with and / or perform restorative and rehabilitative procedures as outlined and assigned by the care plan
    • Measure and record vital signs such as temperature, pulse, respirations, weight, height, and anatomical dimension / circumference
    • Assist moving, positioning and transporting v illagers into and out of beds, chairs, wheelchairs, lifts, etc.; make room checks and verify resident location
    • Assist with the application of slings, elastic bandages, binders, etc
    • Provide range of motion exercises as outlined and assigned by the care plan
    • Work with the v illagers and the Dietitian to create a balanced menu that meets the nutritional needs of the v illagers as well as their preferences for foods they enjoy
    • Prepare and serve meals, between meal, bedtime and other nutritional snacks per the posted menu and stated preferences of the v illagers
    • Keep all areas of the home clean, performing routine housekeeping duties (i.e. changing bed linens, emptying trash, dusting)
    • Launders clothing, linen and other articles
    • Assures v illager calls for assistance are answered promptly
    • Cooperate with the Activity professional to develop and carry out support of the v illager's daily activities, including opportunity for spontaneous activities to occur
    • Maintain confidentiality and rights of villager , family, employee, and house information in compliance with PHI HIPAA policies and applicable regulations
    • Meets with the Clinical Service Team as schedule or needed to discuss v illagers' needs and progress in identifying and correcting problems
    • Observe, monitor, and report to the CST any change in condition
    • Document observed data per policy
    • Will serve in different coordinator roles on a rotating basis. These include Team, Care, Scheduling, Food & Housekeeping Coordinator
    • Attend and participate in Team meetings on a regular basis
    • Attends and participates in orientation programs, on-going training and educational classes
    • Maintains all equipment and appliances in the house by adhering to proper operating guidelines
    • Documents and maintains records necessary to comply with all state and federal regulations
    • Report any problems with equipment or appliances immediately to the maintenance director
    • Adheres to all safety guidelines and processes; and observe, monitor, and intervene in or report to the clinical support team any unsafe conditions;

    observes standard precautions

    • Reports accidents and incidents when they occur
    • Report any abuse or suspected abuse to the Abuse Coordinator i mmediately
    • Follows established smoking regulations and reports violations. (Non-smoking campus)
    • Must be willing to learn and perform duties beyond the traditional scope of practice of a CNA, such as cooking, cleaning, laundry and activities
    • Must be able to adhere to the core value and practices of a Non-Traditional Nursing Home
    • Perform all other duties as requested

    SKILLS AND KNOWLEDGE :

    • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served
    • Must be knowledgeable of laws, regulations, and guidelines pertaining to hospital and nursing facility administrative procedures
    • Must be knowledgeable of computer systems, system applications, and other office equipment
    • Must possess, as a minimum, a 10 th grade education
    • Graduate of a state approved nursing assistant training program and passed a state approved competency examination (written and skills) within 120 days of employment name entered into state nurse aide registry.
    • Six months experience in a long-term care environment preferred
    • Must possess a current, unencumbered, active CNA certification to practice in the state

    PHYSICAL REQUIREMENTS :

    • Must be able to relate to and work with ill, disabled, elderly , emotionally upset, and at times hostile people within the facility.
    • Must meet the general health requirements set forth by the policies of this facility, which may include a mental and physical examination.

    • May be necessary to assist in the evacuation of residents during emergency situations

    WORK ENVIRONMENT :

    • Is involved with residents, personnel, visitors, government agencies / personnel, etc., under all conditions and circumstances.
    • Is subject to difficult and emotionally upset residents, family members, etc.
    • Communicates with other staff, clinical team, and medical director.
    • May work beyond normal duty hours, on weekends, and in other positions temporarily, when necessary.
    • Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
    • Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
    • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
    • May be subject to the handling of and exposure to hazardous chemicals.
