Project Management Manager manages and directs the work of project managers and provides managerial oversight for multiple projects. Monitors project scopes, costs, schedules, staffing, communications, outside vendors, and contractual deliverables. Being a Project Management Manager develops standards, processes, and tools used for effective project scheduling and to set and manage quality targets. Addresses internal or vendor issues that may impede project delivery and develops solutions. Additionally, Project Management Manager tracks at risk metrics and facilitates actions to keep projects on track. Establishes data collection and reporting processes to capture key metrics of project activities and to provide periodic reporting. Requires a bachelor's degree. May require a project management certification. Typically reports to a director. The Project Management Manager manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions.     Title: IL HIV Medical Case Manager

    Reporting Relationship: CM Manager

    Status: Full-Time, Non-Exempt

    Summary/Objective

    The Project exists to eliminate the disparities in health experienced by LGBTQ persons, people living with or at risk of contracting HIV (Human Immunodeficiency Virus), or HCV (Hepatitis C) through education and the provision of services that promote health and wellness.


    The HIV Medical Case Manager is responsible for the case management, including medication and treatment adherence services, of people living with HIV in the Quad Cities and surrounding service areas. The Medical Case Manager is responsible for working in partnership with other community organizations and assessing the biopsychosocial needs of clients. The HIV Medical Case Manager is a member of the Public Health Services Department.


    Essential Functions

    Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

    • Responsible for the case management of a caseload of individuals living with HIV/AIDS and ensure eligibility is maintained for Ryan White Part B services which includes updating income, residency, insurance, and lab work on a bi-annual basis.
    • Develop meaningful rapport and advocate for clients served by maintaining direct communication with community-based organizations and medical providers.
    • Provide crisis intervention and trauma informed care to all clients and patients that seek case management or other services at The Project
    • Complete and monitor biopsychosocial assessments and service care goals for each client.
    • Assist clients with overcoming identified barriers to medication and treatment adherence including, but not limited to, housing instability, inadequate nutrition, transportation barriers, substance use disorders, and mental health needs.
    • Ensure all client services are documented in an accurate and timely manner in the client tracking system identified by IDPH (Iowa/Illinois Department of Public Health).
    • Complete referrals to local community organizations to ensure that Ryan White Part B maintains payer of last resort status.
    • Assist with the facilitation and management of client support group.
    • Develop and maintain close advocate relationship with all clients to include face-to-face contact annually.

    Supervisory Responsibility

    This position has no supervisory responsibilities.


    Physical Demands

    • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work is normally performed in a typical interior/office work environment, with typical office noise and other disruption.
    • While performing the duties of this job, the employee is regularly required to talk or hear. Both standing and sitting are required.
    • Various types of equipment/supplies are used to accomplish the job requirements and include, but are not limited to, calculators, computer keyboards, telephone, printers, scanner, copier etc.
    • The employee may be required to lift and move up to 10 pounds and occasionally lift and move objects up to 25 pounds.

    Position Type/Expected Hours of Work

    Normal hours of work and days for TPQC are Monday through Friday 8:00am to 5:00pm but hours for this position may vary. Working flexibly (some days of the work remotely) is available.


    Travel

    No overnight travel is expected for this position, but local travel is likely to occur weekly.


    Required Education and Experience

    • Associate’s Degree in Psychology, Public Health, Social Work, Mental Health/Behavioral Health, Social Sciences, or 
    • 4 plus years of related experience or
    • Demonstrated knowledge and at least 6 years of related work experience or 
    • Current Illinois licensure as a licensed practical nurse (LPN), registered nurse (RN), or advances practice registered nurse
    • Strong cultural awareness of people living with HIV/AIDS, men who have sex with men, communities of color, and people with substance use issues, and how this may affect or influence service delivery.
    • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical conversations for how The Project can better meet the needs of those we serve.

    Licensure and Certification Requirements

    • Valid driver’s license and current vehicle insurance coverage

    Preferred Training and Experience

    • Special population experience: HIV/AIDS, abuse and sexual assault victims, LGBTQ, Men who have sex with Men (MSM), Hispanic or Spanish speaking populations, African American, refugees or immigrants, substance abuse and/or counseling
    • Certifications in any of the following a plus:

    -Accredited Case Manager – ACM*

    -Certified Case Manager – CCM

    -Certified Social Work Case Manager – C-SWCM

    -Certified Advanced Social Work Case Manager – C-ASWCM

    *ACM requires a state license in Nursing or Social Work
