Additionally, Program Supervisor recommends changes to improve process efficiency and alleviate process inaccuracy and duplication. Requires a bachelor's degree. Typically reports to a manager. The Program Supervisor contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     The Care Coordination Program Supervisor will provide oversight for all care coordination staff and operations to ensure that program services and outcomes provide for a comprehensive system of care delivery.

    Essential Functions:

    Program Administration -

    Oversee the day-to-day operations of the Care Coordination Program, including program planning, implementation, and evaluation.

    Develop and maintain program policies, procedures, and protocols in accordance with federal, state, and local regulations.

    Coordinates and manages client transportation, prescription, and insurance programs.

    Oversees HIV Drug Assistance Program application enrollment, Health Insurance Marketplace, and Medicaid coordination.

    Supervision -

    Responsible for daily supervision and formal reviews of all HIV care coordination staff.

    Coordinate, recruit, manage, and delegate the HIV Care Coordination staff.

    Coordinate and facilitate the training of new HIV care coordination employees and provides training as required for accreditation and contract purposes.

    Coordination and collaboration -

    Serve as the primary point of contact for partner agencies, community organizations, and stakeholders involved in the Ryan White Program to ensure seamless access to care for program participants.

    Work as part of an interdisciplinary case-management, nutrition, housing, mental health, community health, and medical team to meet the needs of the entire person.

    Participant support -

    Provide comprehensive case management and support services to program participants, including assessment of needs, development of care plans, and coordination of services.

    Advocate for the needs and rights of program participants, ensuring access to quality healthcare, social services, and other support resources.

    Conduct regular assessments of participant progress and outcomes, adjusting care plans and services as needed to ensure optimal outcomes.

    Quality and Assurance Compliance -

    Ensure compliance with all applicable regulations, guidelines, and reporting requirements of the Care Coordination Program.

    Develop and implement quality assurance measures to monitor program performance, identify areas for improvement, and ensure the delivery of high-quality services.

    Prepare and submit required reports, documentation, and data to funding agencies and regulatory bodies in a timely manner.

    Other Functions:

    Comply with the agency and Care Coordination Program Policies and Procedures as established by Tulsa CARES.

    Support an environment of health and wellness in accordance with the agency’s philosophy of care.

    Adhere to the Oklahoma State Department of Health Standards of HIV Case Management Care.
