Maintain and coordinate communication among participant, responsible party, staff, and physician as evidenced by:
1. Demonstrate sound judgment by taking appropriate action regarding questionable findings or concerns as evidenced by:
· Follow through with physicians orders
· Follow proper reporting procedures for participant incidents and/or accidents so that follow-up and/or prevention can occur
· Identify problems and/or emergency situations quickly and initiate appropriate action as necessary
· Monitor performance of para-professional staff during shift and immediately address disciplinary problems, and take appropriate action within scope of authority
· Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service
2. Utilize the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of participant care as evidenced by:
· Assess the participant; collect and document current health status
· Document accurate, ongoing assessment of participant status.
· Communicate with responsible party regarding participant needs, the nursing assessment, and recommendations as needed
· Provide personal patient care to provide comfort and well-being to the participant, acknowledging physiological and psychosocial needs
3. Completes regulatory required Nursing assessments and care plans
4. Conducts meetings with Medical Director and notifies Medical Director of changes in plan of care.
5. Conducts meetings with Social Worker weekly and as needed
6. Conducts meeting with Dietician and implements any recommendations.
