Summary: Responsible for evaluating, planning, implementing, coordinating options and services to meet the patient’s health needs through communication and available resources to promote quality cost-effective outcomes.
The LCSW will be part of the care management team and responsible for assisting in the management of the continuum of care and work within an integrated treatment setting. Will provide direct care to clinic patients such as diagnostic assessment, individual therapy, family therapy, group therapy, and crisis intervention to patients, as referred by physicians, and work with the treatment team to promote patient health outcomes.
The LCSW team member will partner with the multi-disciplinary team utilizing best practices and relevant data. Primary elements of the social work role include: assessment of the psychological, financial, and discharge planning needs of the patients and families, providing information, support, counseling, and referrals to other levels of care and appropriate resources for a variety of age-specific populations, including neonatal, pediatric, adolescents, adults, and geriatric.
Requirements: Excellent verbal and written skills to facilitate communication with a variety of medical and technical personnel. Participates in continuing education, i.e., seminars, workshops and educational materials. Computer skills for data collection, comparative analysis, graphic software, word processing and spread sheet usage.
Education Requirements: Must be licensed as a Social Worker with a Masters Degree in Social work. Must have taken and passed the clinical exam for social work.
Experience: All applicants must have completed 4,000 hours of post-graduate, supervised social work experience.
