Job Responsibilities Complies with the facility’s attendance policy requiring regular, reliable, and punctual attendance.
Does Physical Assessment of residents on admission, conducting or coordinating the interviewing of each resident for the resident’s assessment.
Schedules MDS reviews and updates quarterly, annually, and as needed for significant changes in condition. Attends required meetings, e.g., stand-up meetings, in-services, PPS, triple-check, IDTs, and other meetings. Participates in Quality Improvement Program Maintains the MDS and PPS tracking logs.
Interacts with staff within the facility, staff of other departments, and family to coordinate care processes of resident assessment and care planning.
Candidates must have knowledge of PDPM and possess a high level of organization to keep up with skilled patients and long term care residents.
