Position Summary:
Completes schedules for all disciplines. Utilize computer system HCHB (Home Care Home Base). Ensures continuity of caregiver whenever possible. Acts as liaison between patients and staff related to patient schedules. Completes and verifies requested schedule. Verify/Schedule recert visit. Verify/schedule ROC (Resumption Of Care) Oasis Collection. Verify/schedule continuation of services including Scheduling TIF (Transfer Into A Facility) collection. Reschedule declined visit, missed visit, reassigned visit. Process reschedule requests.
Recruitment Requirements:
High School diploma or equivalent.
Minimum 2 years scheduling experience required.
Experience with electronic scheduling a plus.
