Who You Are:

As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC’s, registered nurses, dietitians, and behavioral health clinicians to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity-related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.

What You'll Do:

Practice exceptional medicine within knownwell

• • Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only.
  • Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
  • Coordinates with the broader care team to ensure the patient has the support they need.
  • Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.
  • Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate an MD’s response.

Ensure patients feel known well

• Ensure panel is open to any individual, regardless of their current health status and comorbidities.
• Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey.
• Provide an environment in which patients feel comfortable and free from judgment.
• Offer exceptional encouragement as patients reach their goals.

Coordinate across the collaborative care team

• Escalate any concerns to your dedicated site leadership team.
• Provide clinical guidance to medical assistants and nursing teams as assigned.

Stay current with licensure and appropriate re-certifications.

What You Bring:

• You have an active, unrestricted license to practice in Minnesota, or willingness to obtain.
• You have excellent communication and interpersonal skills.
• You’re dedicated to staying current with the latest developments in primary care and obesity medicine.
• You believe in inclusive, compassionate care.
• You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals. 
• You have high emotional intelligence and know how to make a patient feel at ease – without judgment and without shame.
• You’re willing and excited to try new technologies that are both patient and provider oriented.
• You are excited to work with a collaborative team of physicians, APC’s, health coaches, and other staff.
• We are unable to sponsor visas currently, so you are authorized to work in the United States.
