Physical Therapist - Nursing Home evaluates and treats patients suffering from a physical disability due to injury, disease, or surgery in a nursing home environment. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility. Being a Physical Therapist - Nursing Home works cooperatively with physicians, case managers, and adjustors. Monitors, documents, and reports patients' progress. Additionally, Physical Therapist - Nursing Home may supervise physical therapy assistants, aides, and athletic trainers. Requires an advanced degree of Doctor of Physical Therapy (DPT).     Full Time Physical Therapist Case Manager

    Coverage Area: Santa Cruz County*


    In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment


    Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may may also supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards.     What you’ll do as a Home Health Physical Therapist Case Manager:
    • Perform an evaluation in the home and develop a care plan and work with PTA's to follow the care plan.
    • Help patients with mobility, movement, and help in regaining their independence.
    • Create detailed treatment and care plans for each patient.
    • Communicate and coordinate with patients’ physicians to ensure a quality level of care.
    • Evaluate and report patients’ progress to MD.

    To be a success as a Physical Therapist:
    • Current unrestricted CA Physical Therapist License
    • Current BLS/CPR Certification
    • Valid CA Driver's License
    • Knowledge of local community resources is required.
    • Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
    • Knowledge of state and federal homecare regulatory guidelines preferred.
    • Experience home health care is highly preferred
    • Experience with high acuity patients is a plus
    • Experience working with an interdisciplinary team is highly desired
    • Case Management fundamentals is highly desired

    $120,000 - $135,000 a year
    Wage range Physical Therapist, Level 1: 120,000.00- 135,000.00 plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.  21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match).

    As a practice, the majority of PT Case Managers are hired as PTIs. Once it is determined that an PTI can supervise 2 PTAs, take on a larger case load and assume a higher level of decision making and autonomy, the individual will be promoted to an PTII with a minimum salary of 135,000 plus 20K bonus (Paid Quarterly).

    You will be assigned to a County as a primary location, however you will be expected to be available to cover for vacancies, leave of absences, as well as supporting increased census and client care needs outside of the assigned area. 21st Century Home Health puts forth every effort to accommodate the assigned primary territory, but on occasion, in order to support safe patient care and meet the demands of a growing business, staff may be reassigned temporarily.
