Physical Therapist - Nursing Home evaluates and treats patients suffering from a physical disability due to injury, disease, or surgery in a nursing home environment. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility. Being a Physical Therapist - Nursing Home works cooperatively with physicians, case managers, and adjustors. Monitors, documents, and reports patients' progress. Additionally, Physical Therapist - Nursing Home may supervise physical therapy assistants, aides, and athletic trainers. Requires an advanced degree of Doctor of Physical Therapy (DPT). Typically reports to a manager. Requires a state license to practice physical therapy. Physical Therapist - Nursing Home's years of experience requirement may be unspecified. - Perform evaluations and develop treatment plans
- Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
- Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
- Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
- Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
- Supervise Physical Therapist Assistants as needed.
Qualifications:
- Minimum of a Bachelor's Degree from an accredited Physical Therapy program
- Valid Physical Therapist license in Wisconsin
- A minimum of one year’s work experience as a Physical Therapist is preferred, but not required.
- Experience with dementia is preferred.
