Physical Therapist - Nursing Home evaluates and treats patients suffering from a physical disability due to injury, disease, or surgery in a nursing home environment. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility. Being a Physical Therapist - Nursing Home works cooperatively with physicians, case managers, and adjustors. Monitors, documents, and reports patients' progress. Additionally, Physical Therapist - Nursing Home may supervise physical therapy assistants, aides, and athletic trainers. Requires an advanced degree of Doctor of Physical Therapy (DPT). Typically reports to a manager. Requires a state license to practice physical therapy. Physical Therapist - Nursing Home's years of experience requirement may be unspecified.     Job Summary:

    Physical Therapist will provide coordinated care to clients of all age groups. Plans implements and evaluates client care plans to restore or maintain patient well-being. Plans organize and conducts physical therapy treatment based on the medical referral and evaluation.

    Job Responsibilities:

    • Promote /exemplify the Company mission, vision and values at all times.
    • Perform clinical assessments; identifies patient needs and appropriateness of client.
    • The document, prepare/revise and completes a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.
    • Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).
    • Plan and provide appropriate treatment for neuro-muscular, neurological and orthopedic disorders based on medical referral and therapy assessment.
    • Assist/instruct client/family in transfer techniques appropriate to client abilities. Instruct on use of equipment.
    • Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case conferences/Team Meetings.
    • Monitors assigned cases to ensure compliance with requirements of third party payers.
    • Completes appropriate documentation in a timely manner to assure compliance with company policy.
    • Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.
    • Meet mandatory continuing education requirements of the Agency/licensing board.

    Requirements:

    Experience:

    • Two (2) years’ experience as a PT in a clinical care setting.
    • Preferred, Home health experience.

    Home Health PT Requirements:

    • Current State professional license as a PT in any/all applicable states
    • Current CPR
    • A valid driver’s license and reliable transportation, insurance required.
    • Therapist skills as defined as generally accepted standards of practice.
    • Excellent observational and problem-solving, decision-making and report writing skills.
    • Excellent interpersonal skills and ability to communicate effectively.

    Education:

    • Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
