Physical Therapist - Home Care evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes individualized treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment. Being a Physical Therapist - Home Care maintains patient records by keeping track of goals and progress while adapting treatment plans accordingly. Advises patients and their families on in-home treatment options and what to expect from the recovery process. Additionally, Physical Therapist - Home Care may work with and/or consult with physicians, case managers, and other medical professionals. Requires a doctorate degree in physical therapy. Requires a state license to practice physical therapy. Typically reports to a manager. Physical Therapist - Home Care's years of experience requirement may be unspecified. Performs thorough chart review prior to treatment
Correctly administers physical therapy techniques/modalities in a safe, effective manner, including interchanging treatment media, based on formulated treatment plans and goals
Evaluates patients, utilizing all available information and age specific knowledge, within time frames outlined in policy and procedures
Trains patient/care giver in use of assistive equipment
Effectively and appropriately monitors and adjusts treatment according to patient response to treatment
Completes all documentation required in a timely manner
Recognizes and fulfills all legal responsibilities

Current Physical Therapy License in the State of Wyoming
Current Drivers License
Required Skills
Current BLS certification through American Heart Association
Wound care experience required
