Physical Therapist - Home Care evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes individualized treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment. Being a Physical Therapist - Home Care maintains patient records by keeping track of goals and progress while adapting treatment plans accordingly. Advises patients and their families on in-home treatment options and what to expect from the recovery process. Additionally, Physical Therapist - Home Care may work with and/or consult with physicians, case managers, and other medical professionals. Requires a doctorate degree in physical therapy.     A DAY IN THE LIFE OF A PHYSICAL THERAPIST

    As a physical therapy professional in home health care, you play an essential role in providing our patients with high-quality services that are in accordance with their plan of care. You assist the physician in evaluating the level of functioning in each patient by providing initial and ongoing assessments. After making decisions based on assessments, you help develop or revise the plan of care, prepare clinical and progress notes, and consult with the family and other agency personnel. You then provide the appropriate therapeutic treatments while maintaining the comfort of your patients.

    In addition, you continuously evaluate the effectiveness and outcomes of the care you're administering. Keeping track of your patient's condition, you review their plan of care and apply the appropriate revisions. Using your exceptional communication skills, you coordinate services with other home health care staff members to ensure that patients are receiving well-rounded treatment. At times, you supervise physical therapy assistants and other home health aides as needed as well as evaluate our equipment to ensure optimal levels of functioning.

    You provide all services in accordance with CMS documentation and billing requirements, including functional reassessment requirements. The safety of your patients is of the utmost importance to you, and you take great care to practice infection control by following universal precautions. You also actively recognize and report any forms of abuse according to policy as well as federal and state laws. When needed, you assist with orientation for new hires and participate in any training activities.     QUALIFICATIONS FOR A PHYSICAL THERAPIST

    • Successful completion of a physical therapy education program
    • Current license in good standing with the state of Oklahoma medical board
    • 1 years of physical therapy experience
    • CPR certification or the ability to obtain one within 60 days of being hired
    • Ability to meet the physical demands of the job
