POSITION SUMMARY
Provides clinical and distributive pharmacy services to infant, pediatric, adolescent, adult, and geriatric patients. Incorporates educational, clinical, and technical pharmacy functions to prepare medications, assess and monitor medication therapy. Assists facility staff with medication-related issues and provides drug information to nursing and medical staff.
MINIMUM QUALIFICATIONS
Minimum Education Requirement:
Graduate of an ACPE-accredited school of pharmacy or equivalent. Bachelor of Pharmacy (BS) degree or Doctor of Pharmacy (Pharm.D.) degree required.
Minimum Experience Requirement:
Training in proficiency of age-specific therapies for ages served by the pharmacy.
Minimum License Requirement
Unencumbered license as a Registered Pharmacist in the state of West Virginia or ability to obtain licensure upon hire.
