Pharmacist - Home Care jobs in Texas

Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified.     Job Summary: The Pharmacist directs and supervises the overall operation of the pharmacy contract, including the pharmacy quality assurance program at Integral Care. Coordinates activities to assure that drug therapy with the facility complies with departmental rules and contemporary medical practice. Ensures the compliances of the medication services operation with all applicable federal and state law, including departmental policies and procedures, Texas State Board of Pharmacy regulations, ICF/MR regulations and JCAHO standards. Provide drug information to clients. Assures that patient counseling requirement are met. Provides drug information and education for health care professionals. Oversees center involvement in pharmaceutical company patient assistance programs. 

    Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

    Education:

    Required:

    • Bachelor's degree in Pharmacy.

    License: 

    Required:

    • Current and valid driver's license and a clean driving record. 
    • Licensed as a registered pharmacist in the state of Texas.

    Experience: 

    Preferred: 

    • Five years experience, preferably in a community health setting. 

    Hours: Full-Time 

    Workdays: Monday-Friday. 

    Workhours: 8:00am-5:00pm. 

    Salary: $106,564 - $135,858 annually depending on years of licensure.
