Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified.     PromptCare is seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy.  You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care.     Job Type: Full-Time, 40 hours per week, Mon-Fri 8:00am-5:30pm CST

    Location: Lenexa, KS


    Requirements

    • B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate
    • Active Kansas pharmacist license in good standing 
    • Availability Monday through Friday and participation in the on-call rotation. On-call requirements are approximately one weekend/in 3 weeks. The hours of operation are 8:00am-5:30pm CST


    Job Responsibilities, included but not limited to: 

    • Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management
    • Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness
    • Monitor the patient’s progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team. Most patient contact will be on the phone, and, on occasion, you will meet them in our infusion suite
    • Directly supervise pharmacy technicians and support personnel in the appropriate compounding, packaging, and labeling of compounded sterile product
    • Assist in training and orientation of new employees, performance improvement activities and educational programs
    • Additional licensure may be required in multi-state service areas
    • Go in to IV room and mix medications if needed
    • Other duties assigned


    Physical Demands 

    The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and listen. Requires the ability to lift files, open filing cabinets and bend or stand as necessary. This position requires the ability to occasionally lift office products and supplies of 20 pounds but no more than 35 pounds.
