Pharmacist - Home Care jobs in Kentucky
Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. The pharmacist is responsible for providing daily pharmacy services to all patients of Village Pharmacy in accordance with applicable federal, state, and local standards, guidelines and regulation.
Duties & Responsibilities
- Oversee daily pharmacy operations promoting safety, accuracy, and efficiency.
- Interact with other departments in and outside of Home of the Innocents as part of an interdisciplinary approach to providing care and problem solving daily.
- Dispense medications and other goods as ordered by prescribers daily.
- Clinically evaluate medications being dispensed for safety, effectiveness, and appropriate use daily.
- Supervise and direct pharmacy technicians daily.
- Minimum of B.S. or Pharm.D. degree in Pharmacy from an accredited school or college of pharmacy.
- Must possess and maintain a pharmacist’s license in good standing in the state of Kentucky.
- Basic computer skills required.
- Must be able to learn new computer systems quickly.
- 1-3 years of experience in retail and long-term care pharmacy, preferred.
- This position may require work beyond normal working hours as needed.
- Frequent interruptions may occur.
- This job does not involve any reasonable risk of exposure to blood or body fluids.
- This job does involve threat of repetitive motion injury.
- The job has a 20 lb. minimum weight lifting requirement.
- This job requires moderate prolonged sitting, standing, and bending.
