Pharmacist - Home Care jobs in Escondido, CA
Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. As a Cheer Assistant Care Case Manager, your personality and skills will bring a better quality of life to seniors and adults with disabilities. Your duties on a typical day may include the following:
- Reviewing Shift Notes
- Clocking Caregiver In and Out
- Scheduling Replacement Caregivers for Call-Outs
- Support Caregivers During their Shifts
- Answering Client Questions
- Bringing Ease and Comfort to Our Clients and Caregivers Day
- Documenting all care changes, visits and concerns
- On a Rare Occasion - Visiting a Client
