Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. The Pharmacist functions as an integral member of the hospice care team, working as the clinical pharmacy specialist in collaboration with hospice care physicians and other health care professionals to ensure that medical therapy aligns with the goals of care throughout the course of treatment.
