Position Overview:

AmericaHealth is seeking a dedicated and compassionate Case Manager to join our dynamic team in Idaho Falls working in our Partial Hospitalization Program and Intensive Outpatient Program. We are proud to offer quality care to help those who suffer with mental health issues and substance abuse.

Schedule: Full Time Monday-Friday

Location: On-site

Essential Functions:

The Case Manager interacts with the treatment team in a fast-paced, collaborative work environment that encourages feedback and creative input and provides a variety of client care activities daily.

• Provides case management services to clients receiving substance use treatment and mental health services; supporting clients through admission to discharge
• Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment
• Coordinates effective communication with clients, families, outpatient providers, probation, and other involved agencies to promote the care of the client
• Develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement;
• Supports psychosocial perspective of client's needs to the interdisciplinary team and collaborates with all team members, adjusting plans as needed
• Connects clients with community resources to increase their long term overall well-being
• Provides education and support to clients in group settings
• Documents client’s participation in daily log notes
• Be able to ethically and effectively provide and advocate for client-centered, culturally appropriate, and age appropriate care.
• Execute all required services and documentation in accordance with agency policies and federal/state standards;
• Maintain professional memberships, licenses, accreditation, certifications, etc
• Perform all other duties as assigned by supervisor.

Qualifications:

• Licensed Social Worker
• Bachelor's in Psychology, Sociology, Human Services, or a closely related field required.
• Experience in recovery or mental health setting preferred
• Experience using electronic health records (EHR) preferred
• Case management or case coordination experience preferred
• Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision
• Ability to exercise sound clinical judgment in routine, urgent, and emergent situations
• Proficient computer skills, specifically with all Microsoft office applications
• Able to effectively and efficiently communicate in written and verbal form
• Ability to smoothly and rapidly shift between tasks
• Following de-escalation training, ability to respond appropriately to clients experiencing a crisis

Pay: $45,760.00 - $52,000.00 per year
