Pastoral Care Director - Healthcare develops programs and provides services designed to meet the religious and spiritual needs of patients, their families, and healthcare staff. Ensures that pastoral services are staffed so that chaplains are available to support patients and families facing difficult medical choices. Plans and coordinates with other clergy to conduct interfaith worship experiences. Additionally, Pastoral Care Director - Healthcare may perform the duties of a chaplain. Requires a master's degree in divinity. Typically reports to top management. The Pastoral Care Director - Healthcare manages subordinate staff in the day-to-day performance of their jobs.     WHAT YOU WILL DO: 

     

    Assess and address the spiritual} pastoral, bereavement and additional individual and/or group counseling needs. Develop and implement individualized spiritual and bereavement plans of care in collaboration with local clergy and the interdisciplinary team. Conduct religious ceremonies, bereavement support groups, and educational programs as appropriate Provides training for local clergy in bereavement support and end of life spiritual care.

     

    • Must be available to the patient/family, hospice staff and volunteers, as needed to assess and address the spiritual, pastoral, bereavement and/or the additional individual and/or group counseling needs. Provides spiritual & pastoral counseling services in accordance the plan of care. Complete a spiritual / pastoral assessment as indicated and appropriate as soon as possible after admission.
    • Provides spiritual/pastoral support in accordance with on-going wishes and needs of the patient/ family.
    • Develops individualized spiritual/pastoral plan of care which demonstrates an effort to work in close collaboration with local clergy when desired by the patient; provides spiritual support as needed and defined by the patient/family.
    • Conduct religious services of prayer, worship and rituals for patients/ families as appropriate. Offers patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs, and values.
    • Document counseling provided in the patient's clinical record
    • Works in a team approach with the DG and other qualified professionals, as determined by the hospice, to evaluate patient / family response to care. Works with other professionals in resolving spiritual / pastoral / ethical issues.
    • Participated in IDG conference when plan of care is reviewed.
    • Meets with team members regarding personal spiritual/pastoral issues that may affect their ability to function effectively.
    • Assist in developing and updating the plan of care. Develops the bereavement plans of care and provide bereavement counseling services in accordance with the plan of care. Be available to hospice staff and contracted facility staff for bereavement support. Makes visits/ phone calls to the patient/ family prior to death and is available to family during the bereavement periods.
    • Participate in development of approaches to meet staff counseling needs.
    • Develop and maintain contact with key, targeted referral sources toward the generation of new hospice referrals. Develop new referral sources. Provide referral sources with current information on hospice services.
    • Develop, coordinate and facilitate referrals and intake as needed, a pre-admission visit to interested patients and families to explain our Hospice program.
    • All other duties as assigned.

     

    WHAT YOU WILL NEED TO BE SUCCESSFUL:

    • Ordained, commissioned or credentialed according to the practices of an organized religious group, preferably Episcopal or Presbyterian. 
    • Prioritizes and manages time in an efficient manner and meets productivity standards consistently. 
    • Strong computer skills, including Microsoft and Google applications.
    • Must be able to meet deadlines, work under limited supervision.
    • Excellent verbal and written communication. 
    • Valid state driver’s license. Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers. Proof of motor vehicle insurance. 

     

    PHYSICAL REQUIREMENTS:

     

    • Must be able to climb at least 2 flight of stairs and be able to stand and/or walk for a period of at least 4 hours
