Pastoral Care Director - Healthcare develops programs and oversees services designed to meet the religious and spiritual needs of patients, their families, and healthcare staff. Ensures that pastoral services are staffed so that chaplains are available to support patients and families facing difficult medical choices. Being a Pastoral Care Director - Healthcare establishes policies and procedures to facilitate optimal service delivery and ensure that emotional support, spiritual counsel, and comfort are provided. Evaluates and reviews records of counseling sessions and visitation logs to ensure compliance with professional and ethical standards. Additionally, Pastoral Care Director - Healthcare plans and coordinates with other clergy to conduct interfaith worship experiences. May perform the duties of a chaplain. Requires a master's degree. Typically reports to senior management. The Pastoral Care Director - Healthcare typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a Pastoral Care Director - Healthcare typically requires 3+ years of managerial experience.     Position Summary:
    We have an exciting opportunity to join our team as a Coordinator - Pastoral Care.

    The Hospice Spiritual Care Coordinator will provide support to the Director of Spiritual Care. Is responsible for providing spiritual support, counseling and guidance to patients, families and Hospice Team members in accordance with the philosophy and objectives of the program. Maintains contact with community clergy. Will assist in the provision of bereavement services when indicated and participate in the bi-annual Eve of Remembrance.

    Job Responsibilities:

    • Responsible for implementing & coordinating the provision of spiritual support in keeping with the patient/significant other's desire & belief system.
    • Completes spiritual assessment for each patient, identifies spiritual needs & implements a plan to address those needs.
    • Maintains contact with local clergy to facilitate visitation of congregation members.
    • Visits every Hospice patient at least once (unless refused) & revisits as appropriate.
    • Maintains timely documentation & provides documentation of local clergy involvement in the provision of pastoral care.
    • Makes home visits to provide spiritual support & counsel.
    • Makes arrangements for, participates in or conducts wake, funeral, committal or memorial services when necessary.
    • Coordinates bereavement support for Hospice families & significant others as appropriate. This responsibility is shared with the Hospice Social Work staff.
    • Participates in the bi-annual Day of Remembrance.
    • Attends weekly interdisciplinary team meetings as indicated to contribute to revision of patient's plan of care.
    • Will participate in weekly devotional service at the team meeting when indicated.
    • Communicates with other team members on a daily or as needed basis.
    • On- call availability when necessary.
    • 5-6 hours of appropriate in-service or continuing education programs annually.
    • Participates in the training & continuing education of Hospice volunteers & professional staff.
    • Participates in community education programs & actively seeks opportunities to familiarize local faith communities with Hospice services.

    Minimum Qualifications:

    Bachelor's degree from a regionally accredited college or university or recognized by the New York State Education Department

    One year of training and experience in pastoral/spiritual counseling

    Ability to provide own transportation. Must have a a NYS driver's license

    Ability to read, understand and interpret a map

    Basic computer skills.

    Qualified candidates must be able to effectively communicate with all levels of the organization.

    Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $35,000.00 - $45,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
