Outpatient Care Coordinator supervises the day-to-day activities of the patient relations team of a healthcare organization to deliver patient liaison services and patient-centric care. Coaches staff on best practices for responding to patient and family questions, concerns, and issues. Being a Outpatient Care Coordinator facilitates collaboration with multi-disciplinary teams to make recommendations for improvements to the patient experience. Ensures that escalated problems are addressed and resolved. Additionally, Outpatient Care Coordinator provides and maintains up-to-date informational materials and resources for patients and families. Follows all applicable regulations regarding patient information privacy policies. May require an associate degree. Typically reports to a manager. Working team member that may validate or coordinate the work of others on a support team. Suggests improvements to process, is a knowledge resource for other team members.     Competencies:

    The potential employee must understand target population and specialized needs; possess knowledge of relevant developments in the mental health and substance use (co-occurring) field; possess great writing and communication skills; uses independent judgment; develops effective work methods; adheres to agency policies; takes initiative in work situations; accepts responsibility for actions and displays appropriate professional behavior; provide psycho-education for both the consumer and family or other supports.

    Responsibilities:

    • Assist the Supervisor in ensuring that program provides quality care that meets agency and funding source standards;
    • Transmit agency/program policies and procedures to staff and ensure compliance;
    • Provision of group and individual supervision to program therapists, APN’s and support staff;
    • Analyze staff activity, consumer need and resources; make recommendations to Program Supervisor regarding program needs, directions, and procedures;
    • Review clinical documentation prepared by staff and ensure compliance with contract expectations;
    • Act as a liaison to program and community based entities/supports;
    • Assist the Supervisor with interviewing, selecting, and training new staff;
    • Participate in program meetings, in-service training, quality assurance activities, and other program activities as assigned;
    • Coordinate with prescriber and nursing team for consumers receiving psychiatric and/or MAT medication services;
    • Provision of direct individual, family and group therapy sessions;
    • Assess consumers’ mental, behavioral, and social needs;
    • Collaborate with consumer, consumer’s identified supports, and other providers to develop treatment/service plans;
    • Work as part of a multi-disciplinary team, including case manager and peer recovery specialists;
    • Provide client related reports and evaluations when necessary and maintain a submit reports as needed;
    • Complete managed care or other insurance applications as required by agency on a timely basis;
    • Crisis intervention as needed;
    • All other duties as assigned

    Qualifications:

    • Master’s degree (MA, MS, MSW) in Social Work or related mental health field;

    • NJ license as Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC);

    • Five (5) years’ experience in mental health including 2 years of supervisory experience;

    • Valid driver’s license.
