The Province of St. Joseph of The Capuchin Order
In collaboration with and under the direction of the Director of Clinical Operations, serves friars who reside in a monastery community setting, and independent living environments within Wisconsin, Illinois and Montana. Methods will include using both physical presence as well as remote management technologies. This position will serve as a healthcare coordinator partnering with the friars in a mission to improve and maintain their health. Promote a holistic approach to a healthy lifestyle with the focus on personal responsibility and health maintenance.
ESSENTIAL DUTIES & RESPONSIBILITIES:
· Responsible for overseeing and coordinating plans for the changing and emerging health care needs of the friars.
-Develop individual Wellness Care Plan for friars related to all aspects of health and coordinate plan with Director of Clinical Operations and Local Guardian
-Demonstrate and foster an environment of health care support focusing on overall wellbeing
-Attend and participate in workshops and conferences to develop and maintain current knowledge of wellness, geriatrics, retirement and insurance issues
-Research and maintain information regarding various options of living arrangements (such as assisted living/ nursing homes) in locations throughout Province and ensure smooth transitions for those to various living arrangements in facilities
-Assist friars in managing chronic conditions and acute health problems, provide disease-specific education; provide guidance when specialty care is necessary providing referrals to doctors and community services
-As health care advocate, develop rapport with healthcare staff of clinics, hospitals, nursing homes and other living facilities
-In collaboration with other Province healthcare staff, maintain a medical record for each friar using the standard Province-wide system
-Facilitate and/or accompany friars to medical appointments as needed and when requested. Assist to implement physician orders and follow up to evaluate for compliance, status and appropriate outcome.
-Assist to Initiate and implement a Province-wide Wellness program to include educational materials, presentations and disease-specific information.
-Develop safe medication management for friars needing support
-Comply with Province and ministry policies, procedures, guidelines and standards
SKILLS AND ABILITIES:
-Must be self-motivated and function with minimal on-site supervision.
-Effective communication, problem solving, interpersonal and organizational skills
-Effective computer skills with experience in Microsoft Office
-Must possess strong assessment skills
-Demonstrate the ability to maintain confidential information and preserve the dignity of the friars.

-Ability to travel regularly to Milwaukee WI and Mt. Calvary WI.
-Limited travel to Chicago IL and Montana.

-Must demonstrate a sensitivity to the aging process.
-Must be able to lift and move up to 25 pounds
EDUCATION, TRAINING AND EXPERIENCE:
· Current RN license with at least 5 years of experience with increasing responsibilities
-2 or more years of home care/care coordination/case management
-Experience with geriatric patients.
