What You’ll Need:
- Bachelor’s degree in nursing, Social Work, Community Health or related field required.
- Registered Nurse licensure from the State of North Carolina Board of Nursing or Social Worker licensure in good standing in the State of North Carolina preferred.
- Basic Cardiac Life Support (BCLS) required.
- Spanish language proficiency preferred.
Job Summary:
As a Nurse Education Support Team Coordinator, you'll schedule home visits, manage nurses' assignments, and provide crucial support to families via our public information lines. Your fluency in English and Spanish will ensure all families receive the care they need. Plus, conduct follow-up calls to ensure satisfaction and track referrals.
Job responsibilities include, but are not limited to:
- Maintains compliance with hospital, regulatory, and Joint Commission guidelines and adheres to standards related to education to enhance safe, quality patient care. Engages actively in continuous quality improvement initiatives.
- Demonstrates organizational commitment, ethical and professional behavior by serving as a role model for hospital values to staff, physicians and patients. Maintains appropriate confidentiality of privileged information. Participates in activities to promote professional growth and development.
- Coordinates scheduling of all nurse assignments and manages schedule and client database.
- Assists with in-hospital recruiting as needed:
- Establishes initial contact with families at local birthing hospitals. Successfully engages, recruits, and schedules families for nurse home visits.
- Builds relationships with hospital birth center management and staff including attending meetings, providing educational in-services, and troubleshooting recruitment problems.
- Schedules clients by phone that are missed during in-hospital recruiting.
- Conducts post-visit connection (PVC) phone calls with families, tracks services contacted and received, and provides re-referrals when necessary.
- Distributes appointment cards and PVC surveys.
- Functions as primary public contact to answer questions and reschedule home visits as needed through the public office line in English and/or Spanish, info email and online scheduling tool. Follows up with families in a timely manner, demonstrates good customer service and coordinates coverage when not in the office.
- Functions as an active member of the Nurse Education Support Team by attending all regular staff meetings, Nurse Education Support Team planning meetings, in-service trainings, and other meetings as required.
- Completes other duties and tasks as assigned by Clinical Director and program leadership.
