Nursing Education Coordinator - Home Care is responsible for identifying training needs and planning, implementing, and evaluating the effectiveness of staff development programs within the home care specialty. Coordinates in-service training, continuing education, orientation, competency reviews, and other related training functions. Being a Nursing Education Coordinator - Home Care ensures compliance with all State and Federal guidelines for in-service education and new employee orientation. Requires a bachelor's degree in nursing. Additionally, Nursing Education Coordinator - Home Care requires Registered Nurse (RN) License. Typically reports to a manager or head of a unit/department. The Nursing Education Coordinator - Home Care work is generally independent and collaborative in nature. Contributes to moderately complex aspects of a project.     Job Description

    The MDS Coordinator's Duties and Responsibilities

    • Coordinate and oversee Minimum Data Set (MDS) assessments for each resident utilizing the appropriate forms required by the type of assessment. 
    • Ensure that all assessments are completed and transmitted within the required time frame, and report problem areas to the administrator. 
    • Establish the assessment reference date (ARD), reason for the assessment, accuracy, timely completion, and submission for each assessment. 
    • Initiate the opening and closing of assessments and alert the interdisciplinary team (IDT) as needed. 
    • Develop and implement procedures with the Director of Nursing to inform all assessment team members of the arrival of newly admitted residents. 
    • Participate in functions involving discharge plans as necessary.
    • Monitor residents for changes in condition through facility IDT meetings per facility policies.
    • Perform administrative duties such as completing medical forms, reports, evaluations, studies, etc., as necessary.
    • Maintain and periodically update written facility policies and procedures that govern the development, use, and implementation of the Resident Assessment Instrument (RAI) Minimum Data Sheet (MDS) and care plans. 
    • Develop, implement, and maintain an ongoing quality assurance and performance improvement  (QAPI) program for the resident assessment/ care plans. 
    • Complete electronic submission of required documentation to the state database and other entities in accordance with facility policies.
    • Submit and monitor the nursing home's final validation report to verify assessment submission.
    • Transmit the MDS to the Centers for Medicare and Medicaid Services (CMS) information system for each resident contained in the MDS in a format that conforms to current formatting standards within the prescribed time frame. 
    • Other duties as assigned. 

    Qualifications

     

    • MDS: 1 year (Preferred) 
    • RN (Required)- Good standing and licensed with applicable State Board of Nursing. 
    • Utilization review: 1 year (Preferred)
