Position Summary
The LPN/RN Care Coordinator ? Phone Triage performs clinical specialty care coordination to our patients, providing health advice over the phone through compassion, empathy, and a strong desire to make a difference. The Triage Nurse will assess and triage patient symptoms to assist in obtaining the appropriate level of care and/or self-care advice and documents calls as required. An integral part of our care team, the Nurse Triage team is a trusted liaison for our patients, our providers, and our facility partners ensuring each patient feels comfortable and informed about the next steps in their care.
