  • Duties

    The Women's Health Nurse Navigator/Coordinator (WHNNC) functions as a liaison and is responsible for planning, tracking, and performing a wide range of clinical, administrative, program, and technical functions to support quality case management of gender-specific services for women veterans. The WHNNC possesses the knowledge and skills to effectively apply all aspects of the nursing process with collaborative, interdisciplinary, evidence-based practice, which will enable effective care coordination and implementation of health promotion and disease prevention practices. The following duties include, but are not limited to:

    • Manages and tracks gender-specific care (i.e., maternity care, breast care/mammography, gynecological coordination, infertility, etc.) of patients throughout all aspects of the medical center, including Community Based Outpatient Clinics, as well as community vendors and/or agencies.
    • Assists VA providers and/or nurses, as needed, with the initial consults/orders for gynecology, mammography, non-VA maternity care, and/or other care, to include tracking any subsequent appointments or follow-up, as needed.
    • Educates patients on a variety of gender specific topics, to include importance of routine medical screenings, and authorized VA and community-based resources for infertility, maternal, obstetrical, and infant care.
    • Contacts pregnant veterans on at least a bi-monthly basis throughout the duration of pregnancy and at least monthly during postpartum/lactation period to assess clinical and/or social care needs, facilitating care and services between VA and non-VA care providers. Timing and number of maternity contacts will be in accordance with VHA Directive 1330.03 Maternity Health Care and Coordination.
    • Documents patient care interactions accurately, succinctly, and timely, using appropriate notes, templates, and clinical reminders, in the electronic medical record.
    • Understands mammography reports, Breast Imaging Reporting & Data System (BIRADS), Pap, and other gender specific screening/diagnostic test results.
    • Tracks provider/PACT notification of test results per VHA Directive, Medical Center Policy, and local Standard Operating Procedures. Will notify provider and/or nurse when notification deadline is approaching.
    • Contacts patients who fail to keep women's health appointments, or who are delinquent in obtaining recommended cancer screenings (i.e., mammogram, Pap), to determine and resolve obstacles the veteran may have with keeping appointments.
    • WHNNC will cross cover Mammogram Coordinator, Cervical Cancer Coordinator, and other WH Navigator positions as needed.
    • Incumbent must be self-directed, resourceful, and able to verify productivity with spreadsheets, phone records, or other data, as requested by supervisor, WVPM, or other leadership staff. WHNNC may be required to travel to outlying Community Based Outpatient Clinics (CBOCs), meetings, as needed.

    Qualifications

    Basic Requirements:

    • Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
    Preferred Experience:
    • Preferred experience in women's health with knowledge of gender specific cancer screening guidelines and maternity care.
    • Previous experience in Women's and Maternal Health.
    Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
    • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

    • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

    Environmental Factors: Excessive noise, intermittent, radiant energy, electrical energy, slippery or uneven walking surfaces, unusual fatigue factors (working), working closely with others, working alone, protracted or irregular hours of work, and irregular hours of work.
