Nurse Practitioner - Psychiatric jobs in Illinois

Nurse Practitioner - Psychiatric works in collaboration with physicians to manage care for patients in a psychiatric setting. Additionally, Nurse Practitioner - Psychiatric requires a master's degree. Requires Certified Registered Nurse Practitioner (CRNP). Requires Registered Nurse (RN). Typically reports to a manager or head of a unit/department. Nurse Practitioner - Psychiatric's years of experience requirement may be unspecified.     We are currently seeking a FT Family Nurse Practitioner for Chronic Care Management and Integrative Care for Senior Care Facilities. 

    Schedule:
    Onsite, M-F, 40 hours a week. No on-call or weekends.

    Responsibilities:

    ● Conduct Intake assessments and come up with a care plan.

    ● Be a liaison between the primary care physicians, their nurse practitioners,

    nursing home staff, patient, and their families.

    ● Be open to providing care management services for patients with chronic medical

    and mental health diagnosis.

    ● Create problem lists, measurable goals for chronic medical diagnosis.

    ● Willingness to work with patient’s and their families to provide education based

    on their diagnosis.

    ● Willingness to work with primary care physicians and nurse practitioners.

    ● Willingness to learn about lifestyle medicine coaching.

    ● Willingness to supervise and sign off on lifestyle medicine coaches (Bachelors or

    Masters trained employees) that provide various CCM services that focus on

    topics like sleep, nutrition, physical activity, mindfulness, substance use (alcohol,

    tobacco), and social determinants of health.

    ● Collaborates with various health care providers treating the patients.

    ● Documents in the electronic health record.

     
    Job Requirements:

    ·        Three or more years of experience working in a nursing home. 
    ·        Experience working in Integrated Care is a plus. 
    ·        Active and unrestricted APRN license in the State of Illinois.
    ·        Active DEA in Illinois
    ·        Board Certification
    ·        Willingness to travel to various locations.
