JOB DESCRIPTION
Axzons Home health care nurse practitioner will provide home-based care to chronically ill patients across age groups. They will conduct assessment, diagnose disease conditions, order and provide treatment, and evaluate patient care. They will also provide referral services if necessary. They will work with members of the Axzons health care clinical team and maintain close communication with patients and caregivers.
EDUCATIONAL REQUIREMENTS
Registered nurse preparation, a master’s degree and Nurse Practitioner certification are required.
