ORTHOPEDIC CARE NAVIGATOR
POSITION TITLE: Orthopedic Care Navigator

POSITION SUMMARY:
The Care Navigator manages joint COPC and OrthoAlliance patients to achieve efficient and effective delivery of care utilizing evidence-based medicine. This position serves as the patient’s consistent point of contact and liaison between COPC and OrthoAlliance physicians and physician teams. Duties include processing referrals, priority scheduling, building patient relationships, ensuring care pathway availability, and collaboration with COPC and OrthoAlliance clinicians, leaders, and staff. The Care Navigator addresses any potential barriers that may prevent patients from reaching their treatment goals. This position promotes and facilitates improved clinical outcomes, patient and provider satisfaction and timely use of resources. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Facilitation of Patient Centered Care
- Serve as a liaison between patient, PCP, specialists and other internal and external health care providers to ensure an interdisciplinary coordinated approach to patient care.
- Ensure pre and post program functional assessments are completed and documented appropriately per clinical recommendations.
- Collaborate with OA and COPC to ensure a coordinated approach to care and services for medically complex patients.
- Assist patients in managing the complex hospital, payer and specialist environments to obtain the best outcome.
- Use knowledge of community resources to facilitate achievement of goals.
- Evaluate any identified patient or program concerns in a timely manner and escalate to patient’s care team and/or administrative leadership.
- Assist in providing PCP and specialist offices on-going education about the benefits of the program and how to access services.
Interdisciplinary Practice
- Work in collaboration with healthcare staff, as well as community resources as necessary to facilitate continuity of care.
- Serves as bridge across the clinical setting and functions as patient's consistent point of contact.
- Facilitate referrals to other disciplines and internal health and community based programs as appropriate to improve patient outcomes.
Measurement and Reporting
- Document in the medical record as indicated and designated case management tool accurately reflecting collaborative care planning, interventions and evaluation against defined targets and goals.
- In partnership with management, develop mechanisms to maintain an on-going knowledge of how many patients are receiving care at any given time and the current status of each patient.
- Assists in the collection and reporting of key metrics

Education, Licensures & Certifications
Bachelor’s degree in healthcare related field or equivalent education and experience required
Minimum of 3 years’ experience in a healthcare setting required. Orthopedic experience preferred.
Experience using other software applications such as Excel to track, analyze and report on patient activities preferred.
