    Under general supervision of the Clinical Health Team Supervisor, provides primary health care in the areas of family planning, women’s wellness, and sexual health to clinic clients. Responsible for medical management of health conditions including prescribing and dispensing medications. Performs health assessment, teaching, counseling, direct care, referral and case management to prevent, treat and control diseases or adverse health conditions and promote optimal health in individuals and families with limited access to quality health care. Provides consultation services to other clinic staff.

    This is a part-time (24 hours a week) benefited position based out of the Public Health building in Holland, Michigan. Travel to other County locations as needed. Operation hours are Monday-Friday 8:00am-5:00pm with most Tuesdays 9:00am-6:00pm.

    Set schedule is decided between the employee and supervisor upon hire.

    Essential Job Functions

    The essential functions of this position include, but are not limited to, the following:


    • Provides managed client care by developing and implementing individual treatment plans and performs case management follow-up with other professional and community agencies.
    • Assesses and records presenting illness and risk factors, family health history, review of systems, psychosocial/developmental histories, and cultural information.
    • Performs appropriate physical examination and assessment.
    • Orders or performs appropriate diagnostic and screening tests and collects additional data for evaluation of illness/wellness.
    • Develops differential diagnosis and risk identification through assessment and interpretation of data.
    • Prescribes medications and supervises the dispensing of medication.
    • Provides case management services; develops and monitors care plans and ensures that follow-up services are provided and documented; acts as client advocate; and makes appropriate case referrals.
    • Records patient and treatment data in case notes, and ensures that client case records are properly documented and that confidentiality of client information is maintained.
    • Obtains health history through client/family interviews and chart reviews/compiles evaluation data for quality assurance.
    • Ensures that all client services are provided in compliance with established standards of professional practice and ethics, and OCHD policies, procedures and quality standards.
    • Develops clinical protocols according to the Title X Standards and Guidelines.
    • Performs other related duties as assigned.

    Required Education, Training and Experience

    Master of Science Degree in Nursing (M.S.N.) from an accredited college or university and Board certification as a Nurse Practitioner and eighteen months of progressively responsible and demonstrably successful experience in clinical practice.

    Licenses and Certifications:


    Possession of valid State of Michigan licensure as a Registered Nurse and Specialty certification as a Nurse Practitioner.

    Individuals in this classification provide services to clients in a variety of locations throughout the County are responsible for providing their own transportation.

    Additional Requirements and Information

    Required Knowledge and Skills:

    • Thorough working knowledge of advanced practice of nursing.
    • Working knowledge of medical records documentation standards.
    • Thorough working knowledge of diagnostic testing and interpretation of results.
    • Thorough working knowledge of federal, state and local statutes and regulations governing services for client populations, including Title X Standards and Guidelines.
    • Thorough working knowledge of health care and other public and private human service resources available to client populations.
    • Excellent organizational skills and ability to prioritize the workload.
    • Ability to interact positively and objectively with people from different disciplines and with varying degrees of health knowledge and skills.
    • Excellent written, verbal, and interpersonal communication skills.

    Must be able to perform essential job functions with or without reasonable accommodations, including, but not limited to, visual and/or audiological appliances, and devices to increase mobility.

    Working Conditions:
