WE ARE LOOKING FOR:
Psychiatric Nurse Practitioner, minimum of 2 yrs exp. that lives in and is licensed in Iowa to do work via Telehealth for the following facilities:
72 bed Behavioral Health Hospital 18 bed adult unit, 16 bed adult unit, 18 bed adolescent unit, 10 bed adult unit, 10 bed adolescent unit.
POSITIONS NEEDED: MUST BE COMFORATABLE SEEING ADOLESCENT AND ADULT
- Weekday rounds on approx. 12-16 patients Mon-Fri 7a-7p
- Every other weekend, rounds on approx. 16 patients Mon-Fri 7a-7p
- Weeknight call for admissions and med management Mon-Fri 7p-7a
- Every other weekend call for admissions and med management Sat. and Sun. 7a-7p
*** Please note that these are high acuity, high volume units. We need someone who is very experienced, has no problem with getting documentation done quickly, accurately and timely. The ALOS is 5 days. HCS is the EMR that is used.
Hospital - In-patient Adult Unit- 11 beds and Geriatric Unit - 16 beds
- PMHNP to work every other weekend rounding, call, med management
- PMHNP to work M-F days/nights rounding, call, med management
Hospital - In-patient Adult Unit - 14 beds
- PMHNP to work every other weekend rounding, med management
- PMHNP to work M-F days/nights rounding, call, med management
- Responsible for Daily Patient Rounding On-Call work
What You’ll Do/ Your responsibilities
- Conduct psychiatric assessments/evaluations and diagnose clients according to DSM-5 or ICD-10 criteria
- Formulate a working diagnosis, develop, and implement at treatment plan, and evaluate/modify therapeutic regimens to promote positive patient outcomes
- When clinically indicated, perform assessment scales (e.g., PHQ-9, GAD-7, CSSR-S, etc.), order laboratory tests, review results, and document as required by Forefront Telecare guidelines
- Prescribe, titrate, and monitor psychotropic medications
- Participate in a collaborative approach of patient management using the knowledge and clinical resources of every member of the health care staff to provide the resolution of diagnostic and therapeutic problems and the implementation of effective solutions
- Consult with members of the clinical team as appropriate
- Provide individual and/or family education to increase each patient's awareness of illness and treatment issues
- Participate in administrative or clinical meetings during scheduled hours
- Perform activities and discussions related to coordination of care (PCP communication, calls with primary care practitioner, referral to higher or lower level of care etc.)
- Organize care delivery in collaboration with the interdisciplinary team in a manner that promotes continuity and accountability for patient care
- Promote team approach to patient care through creating and maintaining professional working relationships with physicians and support staff
- Maintain appropriate medical records and complete all necessary clinical documentation in an accurate and timely manner in accordance with Forefront Telecare guidelines
- Video(Zoom) conference with patients who have non-emergency psych needs from your remote, secure location.
- Evaluate and complete notes via EMR system in a timely manner.
- Proficient in patient interview and mental status exams
- Collection of comprehensive histories including diagnostic impressions
- Pharmaceutical management
- Safety evaluations
