Nurse Practitioner - Hospitalist jobs in Vancouver, WA

We are seeking a Nurse Practitioner (NP) to join our team and provide comprehensive care in a Post Acute Care (PAC) facility. You will be responsible for daily rounding, assessing and treating patients, coordinating care with other healthcare providers, and ensuring the delivery of high-quality, patient-centered care.

Responsibilities:

• Conduct comprehensive daily rounds with a physician on assigned patients
  
• Educate and empower patients and their families about their health care needs.
  
• Document care comprehensively and accurately.
  
• Maintain a current knowledge base of best practices in post-acute care.
  

Qualifications:

• Master's degree in Nursing (MSN) from an accredited university.
  
• Current license in the state of Oregon.
  
• Minimum of 1 year of inpatient experience as an NP, preferably in post-acute care or a related setting.
  
• Ability to work independently and as part of a team.
  
• Current BLS and ACLS certification.
