Nurse Practitioner - Ambulatory Care jobs in Elkhart, IN

Nurse Practitioner - Ambulatory Care works in collaboration with physicians and other members of the healthcare team to manage care for patients in an ambulatory care setting. Orders, interprets, and records results of clinical tests and reports results to physicians. Being a Nurse Practitioner - Ambulatory Care prescribes medications and makes recommendations for other therapeutic forms of treatment. Assesses patient condition and may provide treatment using specialized medical equipment. Additionally, Nurse Practitioner - Ambulatory Care works in outpatient settings, responding to high patient volumes in short-term spans and dealing with unpredictable situations and issues. Follows established guidelines and protocols. Requires a master's degree. Requires Certified Registered Nurse Practitioner (CRNP). Requires Registered Nurse (RN). Typically reports to a manager or head of a unit/department. Nurse Practitioner - Ambulatory Care's years of experience requirement may be unspecified.     Job Summary:

    The Nurse Practitioner is a member of the cross-disciplinary team that includes health care providers, medical assistants, nurses, behavioral health providers and other support staff. The Nurse Practitioner supports the mission and values of Maple City Health Care Center (MCHCC) by providing primary care to patients. The position may include call coverage.

    Supervisory Responsibilities:

    • None

    Duties and Responsibilities:

    • Provides primary care for MCHCC patients
    • Utilizes EMR in patient care
    • Utilizes multiple resources for staff communication
    • Other duties that support our shared work, as assigned

    Requirements

    Required Skills and Abilities:

    • Bilingual: English/Spanish preferred, training supported 
    • Trauma Informed Care approach preferred, training supported
    • Motivational Interviewing training preferred, training supported 
    • Demonstrates effective interpersonal skills

    Education and Experience:

    • State of Indiana licensure to practice as a nurse practitioner (ex: RN, APRN, CSR)
    • DEA registration 
    • Board certification (ex: PNCB (Primary Care or dual certified), ANCC)
    • Collaborative practice agreement with MCHCC physicians 
    • BLS required

    Physical Requirements:

    • Ability to lift up to 10 pounds
    • Ability to perform prolonged or extensive standing and walking
    • Ability to perform considerable reaching, stooping, bending, kneeling, crouching
    • Ability to perform prolonged periods of time in front of a computer
