GENERAL SUMMARY:
Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.
