Nurse Practitioner - Acute Care works in collaboration with physicians and other members of the healthcare team to manage care for patients in an acute care setting. Orders, interprets, and records results of clinical tests and reports results to physicians. Being a Nurse Practitioner - Acute Care prescribes medications and makes recommendations for other therapeutic forms of treatment. Assesses patient condition and may provide treatment using specialized medical equipment. Additionally, Nurse Practitioner - Acute Care requires a master's degree. Requires Certified Registered Nurse Practitioner (CRNP). Requires Registered Nurse (RN). Requires Advanced Cardiac Life Support (ACLS). Typically reports to a manager or head of a unit/department. Sub Acute Rehabilitation- Geriatric/Adult Nurse Practitioner

  • Job Summary: We are searching for Advanced Practitioners who will provide on-going medical management for acute/episodic, rehabilitation patients, as well as long term chronically ill patients. Weekend call availability is required. The practitioner will provide on-going medical management and acute interventions for patients in the sub- acute facility setting continuum. The APP will work as a member of the interdisciplinary team to effectively designate processes for meeting the medical, psychosocial and spiritual care needs of the patient, resident. The APP will develop updates and evaluate medical treatment plans, medications, and other necessary care. This position is for one location only.

    Minimum Qualifications:

    Education: Nurse Practitioner (NP) Master’s level program accredited by an appropriate agency recognized by the South Carolina State Board of Medical Examiners (SCSBME).

    Experience: NP with 2 or more years of experience managing patients required. Geriatric experience in an Acute Rehabilitation or Sub-Acute/SNF setting preferred.

    Licensure/Certification: Must be licensed and/or certified by appropriate agencies as required by state law. Must be licensed by SCSBME. Must have a current American Heart Association BLS and ACLS for Healthcare Provider Card. Other certifications may be required depending on the location of the position.

    Knowledge/Skills: Knowledgeable in basic patient care, as well as scope and responsibility of a midlevel provider. Knowledge of the practice to give and evaluate patient care. Knowledge of organizational policies and procedures to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods and techniques of professional on-going patient care. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records and writing reports. Skill in establishing and maintaining effective working relationships with patients, hospital, medical staff and the public. Ability to analyze situations and apply critical thinking. Ability to communicate clearly. Ability to react calmly and effectively in emergency situations. Should be able, after training, to be proficient in electronic medical record

    Other: This position relies on the supervising physician to be available for consultation, provide advice and assume care of patients who have conditions that place them outside the scope of practice. This position must be able to utilize, understand and physically operate clinical equipment as well as office equipment such as computer terminals, telephones, etc. Ability to work under pressure and in emergency situations. Empathy in communication with families. Able to communicate accurately and positively with other health professionals. Good interpersonal skills required. Ability to perform arithmetic calculations with accuracy. Ability to establish priorities and apply common sense approach to patient care. Should have good time management skills and complete work in a timely and appropriate manner

    Pay: $90,000.00 - $120,000.00 per year
