Position Summary:
The Nurse Navigator is a multi-faceted position providing comprehensive services to patients throughout the care continuum working in the role of registered nurse, case manager, and patient liaison. This position is responsible for the clinical, quality, and cost effective outcomes for a caseload of patients, and for collecting, trending, and analyzing aggregate outcomes data for the patient population to be used in identifying best practices and targeting performance improvement efforts. This position requires the advanced knowledge and expertise to identify and implement improvement processes, and the ability to design, direct and implement teaching plans. Work situations require substantial clinical competency and specialized knowledge, judgment, and skill based upon the physical, developmental, psychological, environmental and spiritual principles. This position requires self-direction, motivation, organization, and interpretation of information relating to policies, procedures, and programs. Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers.
Minimum Requirements:
Three to Five years recent clinical experience in area of specialty required
Education/Licenses/Certifications:
Current Washington State License as a registered nurse
Bachelor's degree in Nursing, or obtained within three years from the initial date of hire
Advanced Degree in Nursing (MSN, PhD, DNP, ARNP) preferred
Certification in assigned area preferred
Compensation Range:
N04 - Min: $52.51-Max: $77.21
