    Position Summary:

    • The Nurse Navigator coordinates care with patients, families, and healthcare providers. Acts as a liaison between the physician, professional healthcare staff and other involved parties. Helps to collaborate with the healthcare team providing a seamless transition by ensuring appropriate and comprehensive health services across the continuum of care. Participates in assessing, planning, implementing and evaluating health. The palliative care nurse navigator is responsible for addressing the needs of the whole person: body, mind and spirit. Responsible for linking patients with financial resources, education material, transportation services and other hospital and community based resources. Report outcomes and participate in quality improvement initiatives.

    Credentials / Education:

    • Current and Valid License as a RN in TN Required
    • Specialty Nurse Certification (Palliative Care) Preferred
    • Bachelor’s in Nursing Preferred
    • 3 Years Clinical Nursing Experience Required
    • 2 years Palliative Care Experiences Preferred
