Nurse Manager - Phone Triage manages the day-to-day operation of a nurse triage call center and implements established policies, procedures, and service models. Uses performance and quality standards to achieve and maintain required care delivery levels. Being a Nurse Manager - Phone Triage monitors operations with call data analysis to identify trends, measure productivity, and respond to increases in demand for services. Conducts recruitment and selection activities to maintain optimum levels of nursing staff. Additionally, Nurse Manager - Phone Triage ensures compliance with nursing staff standards for competencies, assessments, licensure, certifications, and other annual training. Responds to escalated problems and may provide patient care evaluations and recommend responses. Typically requires a bachelor's degree in nursing. Requires Registered Nurse (RN). Typically reports to a director. The Nurse Manager - Phone Triage manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Nurse Manager - Phone Triage typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required.     Position Summary:

    Provides after hours triage services to Hawaii Care Choices (HiCare) patients in the Hospice and Palliative are programs. Provides direct patient care as needed, includes telephonic and telehealth visits. Collaborates with the Clinical Operations Manager and Director of Clinical Services to ensure excellent patient care services.


    Duties & Responsibilities:


    —

    • Answers after hour phone calls. Includes holidays and weekends.

    • —

      Connects with patient and family utilizing telehealth tools. Includes telehealth visits ad response to all Hawaii Care Choices programs (Hospice & Palliative).

    • —

      Conducts telephonic assessments of patient and family needs. Includes all Hawaii Care Choices programs (Hospice & Palliative).

    • —

      Provides excellent customer service in accommodating patient/family needs.

    • —

      Provides symptom management and resolving complex issues.

    • —

      Provides call resolution to problems, needs and issues that may arise.

    • —

      Works with Clinical Operations Manager and Director of Clinical Services in ensuring excellent patient care.

    • —

      Provide direct patient care including comprehensive, routine, admissions and as needed visits to patients.

    • —

      Practice clinical standards that comply with HiCare’s mission, vision, values, and policies and procedures.

    • —

      Communicates in an effective and timely manner with team members and staff.

    • —

      Ensures the safe custody and administration of drugs in accordance with the Standards of Practice.

    • —

      Ensures adherence with performance metrics – reports all concerns to the immediate supervisor.

    • —

      Actively seeks out and implements evidenced based practices.

    • —

      Attends to own personal and professional development needs as identified in personal development plan.

    • —

      Maintains a sound working knowledge of the Hospice Conditions of Participation (COPs).

    • —

      Upholds and maintains the organization's cost containment efforts without jeopardizing the quality of hospice care.

    • —

      Provides education and training to patients, care givers, volunteers, and other team members as deemed necessary.

    • —

      Participates in meetings and committees as requested.

    • —

      May help with admissions.

      Ability to provide unsupervised health care services in private residences, along with handling and administering controlled and prescribed medications.

      Ability to coordinate care with the interdisciplinary team.

      Excellent verbal communication skills with positive attitude focus.

    • —

      Interest and desire to give care with a special sensitivity to the needs of the dying.

    • —

      Maintenance of adequate educational and experiential level to meet the needs of the patients and care givers and regulations established by state and federal laws.

      Possess intermediate knowledge or aptitude to operate a computer, knowledge of basic software programs.

    • —

      Strong desire to work as part of a highly interactive team.

    • —

      Should be able to prioritize and delegate comfortably.

    • —

      Must be a strong patient and family advocate.

    • —

      Must be thorough and detail oriented.

    • —

      Use ethical principles in decision making and practice.

    • —

      Comfortable in maintaining professional boundaries.


    Educational and Experience Requirements

      —
    • Graduate of a registered nursing curriculum ADN required; BSN degree or higher preferred.

    • —

      2 years’ experience in a healthcare environment or equivalent position.


    Hours

      —
    • Regular full-time employee. After business hour shifts (may start mid-day at times). Schedule determined by Supervisor.

    • —

      Duties require after hour work including weekends and holidays, as needed.

After receiving a final offer of employment, all prospective HiCARE employees are required to be fully vaccinated against COVID-19.
