Purpose of this position
The purpose of this position is to ensure that residents' needs are accurately assessed to identify the appropriate level of care to be provided by the appropriate payer.
Qualifications
Education: Associate degree
Licensure or Certification: Licensure as a licensed vocational nurse is required. Prefer registered nurse licensure.
Experience: One year as a long-term care nurse with MDS duties required.

Reimbursement and Care Plan Coordinator Performance Standards
Review resident pre-admission and admission information.
Review resident assessments for accuracy, completeness, and timeliness.
Coordinate with other disciplines for MDS completion.
Review resident RUGS level of care determinations for accuracy.
Compare resident assessments with skilled care documentation to identify concerns.
Review resident assessments with billing statements for all payer sources for accuracy.
Submit MDS and monitor electronic submission of assessments to CMS in a timely basis.
Interview residents, residents' representatives, and others for information regarding the MDS.
Examine residents to collect information related to the MDS.
Review medical records related to the residents' health status to collect information for the MDS.
Enter data into the computer program for MDS assessments.
Create the care plan schedule, notify staff, residents, and responsible parties.
Coordinate and supervise the care plan meeting. Complete care plan documentation
Utilize MDS and CAA data, risk assessments, and consultant notes for care plan creation.
Create 24 hour, 36-hour, 21-day care plan, quarterly care plan updates. and care plan updates based on new conditions or incidents.
Participate in the quality assurance program for the nursing department.
Perform other related duties as directed by the Director of Nurses or Administrator,
