MDS (Minimum Data Set) nurse is a registered nurse (RN) who conducts federally mandated assessments of the patients at a long-term health care facility. MDS nurses are responsible for collecting integral data and compiling it into a thorough report for further research at their facility.
The primary purpose as an assessment tool is used to identify resident care problems that are addressed in an individualized care plan, data collected from MDS assessments, and monitoring the quality of care provided to the residents.
Being a licensed Registered Nurse
Having clinical experience in a healthcare setting
Demonstrating basic computer skills
Obtaining a Bachelor’s in Nursing
Showing proficiency in MDS 3.0
Demonstrating knowledge of state and federal regulations
Update and complete resident’s discharges and re-entries into the computer
* Attend all morning stand-up meetings and gather information on resident’s status
* Attend admission meetings and gather data
* Attend all staff in-service meetings
* Gather information from RN, LPN, DCW and Residents
* Make Monthly Care Plan Schedules and give updates to all interdisciplinary team members
* Complete the user defined assessments: Pain, Aims, Bowel and Bladder Assessments
* Complete an MDS Note with each assessment done
* Review interdisciplinary team members notes and user defined assessments for completion
* Enter the assessments in the computer according to the MDS schedule
* Update and modify the care plans according to MDS schedule and prn
* Attend all care plan meetings and complete a Care Conference note in the computer
* Identify problems/solutions with IDT members
* Update orders from medical records in the care plan section weekly and prn
* Close, Batch and Transmit completed assessments to VA Austin weekly
* Attend weekly wound/skin/nutrition/fall meetings
* Attend quarterly QA/QI meetings and identify triggered areas
* Dining room duties as schedules
* On call as scheduled
* Work a medication cart as needed
* Complete information daily on assessments open and take to stand-up meeting
* Maintain updated file on each resident with 18 months of MDS assessments
* Enter CNA Care Plan Task in computer with each resident admitted and keep updated prn
