Job Summary: We are seeking a skilled and experienced MDS Coordinator to join our healthcare team. The MDS Coordinator will be responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments for our patients. This role requires strong attention to detail, knowledge of medical documentation, and the ability to work collaboratively with healthcare professionals. Duties: - Conduct comprehensive assessments of patients' medical conditions and needs - Collect and analyze patient data to complete accurate MDS assessments - Ensure compliance with all regulatory requirements, including HIPAA guidelines - Collaborate with interdisciplinary healthcare team members to develop individualized care plans - Monitor and update patient records as needed, ensuring accuracy and completeness - Provide guidance and education to staff regarding MDS documentation and coding guidelines - Participate in quality improvement initiatives to enhance patient care outcomes - Assist in the coordination of patient care activities, including triage and physical examinations - Provide direct patient care as needed, including administering medications, performing basic life support, and assisting with sutures Skills: - Strong knowledge of MDS assessment process and coding guidelines - Proficiency in medical documentation and record keeping - Excellent communication skills, both verbal and written - Ability to work collaboratively with interdisciplinary healthcare team members - Knowledge of tube feeding procedures and protocols - Familiarity with basic life support techniques - Ability to perform physical examinations and triage patients effectively - Attention to detail and strong organizational skills We offer competitive compensation packages, including benefits such as health insurance, retirement plans, and paid time off.
