* As an RNAC MDS Coordinator, you will perform the following duties :
* Develop preliminary and comprehensive assessments of the nursing needs of each residents, utilizing the forms required by current rules or regulations and Facility policies.
* Ensure that all members of the assessment team are aware of the importance of completing accurate, complaint, and timely resident assessments. Including the implementation of RAPs and Triggers.
* Coordinating and conducting the development and completion of the resident assessment and care plan in accordance with the requirements of the state and policies and goals of the facility.
* Assists with ensuring that documentation at the facility meets Federal, State, and Certification guidelines.
* Ensure the resident and Discharge Planning Coordinator in completing the care plan portion of the resident's discharge plan.
* Monitor the facility's QI and QM report.
* Develop the schedule of activities required for the resident assessment and plan of care.
*Ensure that all assessments are completed and transmitted in a timely manner. Report problem areas to the Administrator.
*Develop and implement procedures with the Director Of Nursing Services and interdisciplinary team to inform all assessment team members of the arrival of newly admitted residents.
*Ensure that each portion of the assessment is signed and dated by the person completing that portion of (MDS)
*Assist Therapy in interviewing residents to evaluate daily living skills and capacities and determine extent of abilities and limitations
Job Requirements:
* Registered Nurse with a current DC License.
* Must have a minimum , two (2) years of experience as a supervisor in a hospital, nursing care facility, or other related care facility.
*Must have, as am minimum, six (6) months training experience in rehabilitative and restorative nursing practices.
*Must have (2) years of experience as an RNAC MDS Coordinator.
