- Conduct comprehensive assessments of patients' medical status
- Coordinate with healthcare providers to ensure accurate and timely completion of Minimum Data Set (MDS) documentation
- Utilize electronic health record systems for data entry and retrieval
- Collaborate with interdisciplinary teams to develop individualized care plans
- Monitor and evaluate patient care outcomes
- Stay updated on regulatory requirements and changes in healthcare policies

Qualifications:

- Proven experience in medical coding, utilization review, and case management
- Familiarity with primary care practices and HEDIS measures
- Knowledge of DRG coding systems and medical management principles
- Understanding of physiology and ICD-10 coding standards
- Strong communication and organizational skills
