Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     Working Conditions

    • Work throughout the nursing unit.
    • Sit, stand, bend, lift, and move intermittently during working hours.
    • Subject to frequent interruptions.
    • Involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
    • Subject to hostile and emotionally upset residents, family members, personnel and visitors.
    • Work beyond normal working hours, and in other positions temporarily, when necessary.
    • Subject to fall, burns from equipment, odors, etc., throughout the workday.
    • Subject to exposure to infectious waste, diseases, conditions, etc., including the AIDS and Hepatitis B viruses.

    Education

    • Must possess a current, unencumbered license to practice as a RN or LPN in Kansas.

    Experience

    • One (1) year experience as a nurse, including some medical-surgical experience.
    • Awareness of the needs of mental health residents.
    • Advanced training in the MDS process.

    Specific Requirements

    • Must be able to read, write, speak, and understand the English language.
    • Must possess:
    • The ability to make independent decisions when circumstances warrant such action.
    • The ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
    • The ability to calculate figures related to medication dosages and amounts such as percentages and area.
    • The ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
    • The ability to write reports.
    • The ability to effectively present information and respond to questions from co-workers, residents, families, visitors.
    • Leadership and supervisory skills and the willingness to work harmoniously with professional and non-professional personnel.
    • The ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
    • The ability to organize, develop, implement and interpret the goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
    • Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to long-term care.
    • Patience, tact, cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
    • The ability to accurately assess and relay information concerning a resident's condition.
    • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
    • Must be able to assist with CPR.
